This calculator is for educational planning only. It cannot determine whether a treatment service is covered, medically necessary, in network, authorized, or available. Ask your insurer and treatment provider for an actual benefits and cost estimate.

An in-network limit cannot automatically be applied to out-of-network services, noncovered costs, premiums, or balance bills. Some plans have separate limits.

How to interpret this calculator

Deductible

The portion of applicable covered expenses you pay before a plan typically shares costs.

Coinsurance

Your percentage of remaining insurer-allowed covered expenses after the deductible.

Copayment

A fixed payment that varies by plan and service. This tool adds it only if you explicitly confirm it applies alongside the other modeled charges.

Out-of-pocket limit

A cap on qualifying cost sharing for covered services. Many excluded, out-of-network, and balance-billed amounts do not count.

Method: The model first allocates the applicable allowed amount toward the remaining deductible, then calculates coinsurance on the remaining amount. An additional copay is counted only when explicitly selected. An applicable in-network limit can reduce qualifying cost sharing, and separately entered noncovered costs are added afterward. Real plans may use different service-specific rules, bundled charges, multiple claim lines, or authorization criteria.

Not a quote: Results do not confirm benefits, coverage, authorization, clinical appropriateness, or a provider's actual charges. Out-of-network balance billing is not estimated by this calculator.
